Adjustment Disorders & Life Transitions

Major Life Changes Can Produce Clinical-Level Symptoms. That Does Not Mean You Are Weak — It Means You May Need Support.

D&Y provides psychiatric evaluation for adults whose response to a major life stressor — job loss, divorce, bereavement, relocation, or cumulative change — is significantly impairing daily functioning in ways that are not resolving on their own.

An adjustment disorder is a clinical diagnosis that recognizes significant emotional or behavioral symptoms that develop in response to an identifiable stressor and go beyond the expected reaction — impairing work, relationships, or daily functioning. D&Y evaluates whether what you are experiencing reflects an adjustment disorder, a major depressive episode triggered by the stressor, or another clinical condition that the transition has unmasked.

What It Is

When a Normal Response to a Hard Situation Becomes a Clinical Condition.

Adjustment disorders are diagnosed when emotional or behavioral symptoms develop in response to a specific identifiable stressor — within three months of its onset — and produce either marked distress out of proportion to the severity of the stressor, or significant impairment in social, occupational, or daily functioning. The stressor does not have to be catastrophic to produce a clinical response. Job loss, divorce, a significant health diagnosis, the death of a relationship, or the accumulation of smaller changes can all qualify.

What distinguishes an adjustment disorder from ordinary difficulty is the level of functional impairment — not the severity of the stressor. A person who is struggling to work, to sleep, to maintain relationships, or to manage basic responsibilities in the aftermath of a significant life change is experiencing something that warrants clinical attention — regardless of whether the circumstances feel "bad enough" to deserve it.

Adjustment disorders are also diagnostically important because they sit at the threshold of several other clinical conditions. A significant life stressor can trigger a major depressive episode in someone with a prior mood history, unmask ADHD that was previously compensated, or exacerbate anxiety that was previously managed. D&Y evaluates what is actually happening — whether the presentation is truly an adjustment response, or whether a concurrent condition needs to be addressed alongside it.

Common Presenting Stressors

Major Transitions That Bring Adults to D&Y for Evaluation.

Job loss or involuntary career change
Divorce or end of a significant relationship
Death of a close family member or friend
Significant health diagnosis — self or close family
Major relocation or life disruption
Retirement and loss of professional identity
Parenting transitions — empty nest, new child
Business failure or financial crisis
Career change or return to workforce
Accumulation of multiple simultaneous changes

The stressor does not have to be on this list. If a significant change in your life is producing symptoms that are impairing your functioning — that is worth evaluating.

Our Approach

Evaluation That Distinguishes What Is Situational From What Is Clinical.

Our adjustment disorder evaluation is a structured clinical interview that assesses the specific stressor, the onset and pattern of symptoms, the degree of functional impairment, and the full diagnostic picture — specifically evaluating whether the presentation reflects an adjustment disorder or a concurrent clinical condition that the stressor has triggered or worsened. He does not pathologize ordinary grief, loss, or difficulty — but he does evaluate whether what is present meets the clinical threshold that warrants treatment.

Treatment for adjustment disorders at D&Y is tailored to what the evaluation finds. Some presentations benefit primarily from medication management to stabilize mood, sleep, and anxiety during the transition period. Others benefit more from referral to a therapist for structured support. Your provider recommends what the evidence and the clinical picture support — including the honest assessment that psychiatric medication may not be the primary intervention needed.

Full Evaluation

60–90 minutes. The specific stressor, symptom onset and pattern, and degree of functional impairment are assessed. A complete differential is conducted — concurrent conditions identified alongside the adjustment response.

Honest Recommendation

Is this primarily adjustment? Is there a concurrent condition — depression, anxiety, burnout — that the stressor has triggered or worsened? What does treatment actually look like for this specific presentation? You receive a clear answer.

Treatment or Referral

Medication management if clinically indicated. Therapist referral with a specific recommendation if more appropriate. D&Y coordinates both — and is direct about which the evidence supports for your situation.
