You Do Not Have to Call It Trauma to Recognize That Something in Your History Is Affecting How You Function Today.
D&Y provides psychiatric evaluation and care for adults whose past experiences — childhood trauma, prolonged relational harm, or a history that never felt safe — are contributing to the depression, anxiety, emotional dysregulation, or relationship difficulties they are living with now.
Trauma does not require a single dramatic event to leave a clinical imprint. D&Y evaluates the full history and the current presentation together — determining what is driving the symptoms, what psychiatric care can address, and what requires coordination with trauma-specialized therapy.
Unaddressed Trauma Does Not Stay in the Past — It Organizes the Present.
The long-term effects of childhood trauma, chronic relational harm, and complex traumatic exposure do not look the same in every adult. Some look like depression that has never quite responded to treatment. Some look like anxiety that is always scanning for threat even when no threat is present. Some look like a persistent difficulty in relationships — a pattern of either too much closeness or too much distance, or an inability to trust that stability will last. Some look like an identity that has never felt fully coherent, or a chronic sense of shame that does not connect clearly to any present-day cause.
What these presentations share is an underlying nervous system organization that was shaped by experiences that required it to be consistently alert, defensive, or adapted to unpredictability. That organization made sense in the context where it developed. In adult life — in relationships, in workplaces, in situations that are objectively safe — it often creates exactly the difficulties the person is now seeking help for.
Many adults with significant childhood trauma histories have never connected that history to their current clinical presentation. They have been treated for depression, for anxiety, for relationship problems — without anyone asking the questions that would reveal the common thread. The treatment was not wrong. It was incomplete.
D&Y evaluates trauma history as part of every relevant assessment — not to re-traumatize, not to require extensive disclosure, but to understand what is driving the current presentation and build a treatment plan that accounts for it.
"Adults with complex trauma histories are often the most treatment-resistant patients in outpatient psychiatry — not because they are hard to help, but because no one has evaluated the full picture. When the picture becomes clear, the path forward usually does too."— Denis Grigorov, PMHNP-BC
PTSD and Complex PTSD Are Related but Distinct — and the Treatment Approach Differs.
Understanding which framework best fits your experience shapes how your provider builds your evaluation and treatment plan.
Post-Traumatic Stress Disorder
- Typically develops from a discrete traumatic event or series of events
- Core clusters: intrusion, avoidance, hyperarousal, and negative cognitions
- More likely to have a clear onset point that the person can identify
- Responds well to medication management and evidence-based trauma-focused therapy
- Often addressed in the veteran, first responder, and acute-trauma contexts
Complex PTSD
- Develops from prolonged, repeated, or inescapable trauma — particularly in childhood
- Emerges within close relationships: caregivers, partners, or institutions
- Additional features: pervasive emotional dysregulation and chronic negative self-perception
- Disrupted relational patterns that follow the person into adult life
- Often presents as depression, personality difficulties, or chronic anxiety with no clear traumatic event identified as the cause
Trauma History Frequently Drives or Complicates Other Conditions. D&Y Evaluates the Connection.
If you have been treated for any of the following without lasting results, unaddressed trauma history may be a factor that has not yet been fully explored.
Depression
Childhood trauma significantly elevates lifetime risk for depression and is one of the most common factors in treatment-resistant presentations. Depression driven by unaddressed trauma often requires a different treatment approach.
Learn moreAnxiety
Chronic hypervigilance, emotional reactivity, and difficulty tolerating uncertainty are common in adults with trauma histories — and frequently present as anxiety that does not fully respond to standard treatment.
Learn moreBipolar & Mood Disorders
Emotional dysregulation from complex trauma can present similarly to bipolar disorder and is frequently misdiagnosed. D&Y evaluates for both before making any treatment recommendation.
Learn morePTSD
When the trauma history is more discrete or the presentation aligns with PTSD symptom clusters specifically, D&Y evaluates and treats under the PTSD framework with full clinical rigor — particularly for veterans and first responders.
Learn morePsychiatric Evaluation That Asks the Right Questions and Coordinates the Care.
Our trauma-informed evaluation is a structured clinical interview that assesses the history thoughtfully — without requiring the patient to provide extensive detail about traumatic events — while building a complete picture of how the history and the current presentation connect. He does not require patients to use the word "trauma" to receive care for a presentation that reflects one.
Treatment at D&Y for trauma-related presentations includes psychiatric medication management for the symptoms that respond to it — depression, anxiety, hyperarousal, sleep disruption — and active referral to trauma-specialized therapists for the processing and relational work that medication cannot produce. D&Y refers with specific, trusted recommendations and communicates with therapists directly with patient consent. He does not leave patients to navigate the handoff alone.
The goal of treatment is not erasure of the history. It is reduction of the current cost — the depression, the hypervigilance, the relational patterns, the emotional dysregulation — to something that no longer determines the shape of the person's present life inoff. Continue seamlessly from exactly where you stopped. ```html the ways it currently does.
