Why D&Y

A Psychiatric Provider Your Patients Will Actually Stay With.

The most common reason therapists and PCPs stop referring to a psychiatric practice is not clinical — it is operational. The patient goes to the evaluation, gets a prescription, and the referring provider never hears what happened. D&Y practices differently. With patient consent, we communicate directly with referring providers, share the clinical picture, and make sure the treatment plan is aligned across care relationships.

For patients in therapy, this means your provider and the referring therapist work from the same diagnostic framework — not two separate understandings of the patient that never intersect. For PCP referrals, it means we send a consultation summary and remain accessible for questions as medication management progresses. For VA Community Care referrals, it means the patient receives the coordinated, personalized care that the VA system cannot always provide within its own infrastructure.

The 60–90 minute first appointment is the structural foundation of this. We do not make medication decisions based on a 15-minute intake. We build a complete diagnostic picture — one that the referring provider can trust as a real evaluation, not a rubber stamp.

Denis's Full Clinical Background

Board-Certified PMHNP

Denis Grigorov, PMHNP-BC — board certified through the American Nurses Credentialing Center. Psychiatric Mental Health Nurse Practitioner training at California State University, Long Beach.

VA-Trained & Military Veteran

12-month psychiatric residency at the VA Greater Los Angeles Healthcare System. 10 years as a commissioned officer in the U.S. Air Force. Particular depth with veteran and first responder populations.

Conservative, Diagnosis-First Prescribing

D&Y does not prescribe before evaluation. Minimum medication necessary to achieve meaningful improvement. Every decision explained to the patient and communicated to the referring provider.

Insurance Access

Most major commercial plans, Medicare, VA Community Care, and TriWest accepted — making D&Y accessible to the broad range of patients referring providers serve.

Scope of Practice

Conditions D&Y Evaluates and Treats in Referred Patients.

D&Y provides outpatient adult psychiatric evaluation and medication management for the following conditions — all via telehealth, all in adults 18 and older located in California.

D&Y does not provide: IOP or partial hospitalization, inpatient psychiatric care, crisis intervention or emergency psychiatric services, forensic or court-ordered evaluations, complex substance use withdrawal management, long-term disability documentation, or care for patients under 18. If a referred patient requires a higher level of care, we will communicate this directly and assist with an appropriate transition.

How Referrals Work

A Straightforward Referral Process That Respects Your Time and Your Patient's.

Five steps. No administrative maze. The office handles insurance verification and patient outreach — you provide the clinical context and we take it from there.

Contact the Office

Submit a referral by phone, email, or through the contact form. Include the patient's name, a brief description of the presenting concern, and your contact information. That is all that is needed to initiate the process.

Insurance Verification

D&Y verifies the patient's insurance coverage before scheduling. The patient is contacted directly to complete intake paperwork and confirm their first appointment. The referring provider is not responsible for this step.

60–90 Minute Psychiatric Evaluation

D&Y conducts a comprehensive evaluation at the first appointment — not a brief intake. The patient receives a working diagnosis, a clear explanation of the clinical reasoning, and a structured treatment plan with defined next steps.

Coordination With the Referring Provider

With patient consent, we communicate the evaluation findings and treatment plan to the referring provider. Ongoing coordination is maintained throughout the treatment relationship as clinically appropriate.

Ongoing Medication Management

Follow-up visits every 2–4 weeks early in treatment, transitioning to monthly once stable. Referring providers are kept in the loop as the treatment progresses when coordination is indicated and consent is in place.
