ADHD Evaluation & Management

Adult ADHD Often Goes Unrecognized for Years. We Start With a Thorough Evaluation, Not a Quick Assessment.

D&Y Integrated Psychiatry provides structured ADHD evaluation and medication management for adults across California — diagnosis first, prescribing second, explanation always.

Adult ADHD looks different than it did in childhood, and it is frequently missed, misdiagnosed, or undertreated in high-functioning adults who have built workarounds for decades. D&Y conducts a structured ADHD evaluation that distinguishes true ADHD from anxiety, mood disorders, and other presentations that can produce nearly identical symptoms.

Who We Evaluate

Three Types of Adults Come to D&Y for ADHD Care.

Wherever you are in your diagnostic journey, you are in the right place. Our evaluation is built for all three of these situations.

You Have Never Been Formally Evaluated

You have suspected ADHD for years. You have read the lists. You recognize yourself in every description. But you have never had a proper evaluation — either because you were not sure you would be taken seriously, or because you did not want a 10-minute checklist and a prescription. Our evaluation is thorough enough that you will leave with an answer you can actually trust.

You Were Diagnosed, But Something Is Off

Maybe you were diagnosed quickly. Maybe the medication worked at first and then stopped. Maybe you have never been sure the diagnosis was right. Your provider will review your history, evaluate your current presentation, and give you a clear clinical picture of what is actually happening — separate from what the prior chart says.

You Are Skeptical You Have ADHD

You have been told you might. You are not convinced. You are specifically looking for a provider who will evaluate you rigorously rather than confirm a suspicion. D&Y approaches every evaluation as an open diagnostic question. If the evidence supports ADHD, he will say so. If it does not, he will say that too — and help you understand what might actually be driving your symptoms.

Why This Matters

Adult ADHD Is Not Just Childhood ADHD That Grew Up.

"Structured evaluation before any prescribing. If the evidence supports ADHD, we treat it. If the symptoms point somewhere else, we treat that. Either way, you leave with clarity."

— Denis Grigorov, PMHNP-BC

The clinical presentation of ADHD in adults often bears little resemblance to the hyperactive child who could not sit still. High-functioning adults with ADHD have typically developed compensatory strategies over decades — they work harder than everyone else to produce the same output, run on chronic stress and deadline pressure as their primary focusing mechanism, and carry a persistent sense that they are underperforming relative to their actual capability.

The diagnostic challenge is that anxiety, depression, bipolar disorder, sleep disorders, and burnout can all produce symptoms that closely overlap with ADHD. Difficulty concentrating is not a diagnosis — it is a symptom that requires a full differential, and many providers skip that step. The result is patients on medications who actually have a different condition, and patients on three medications who have never had either condition properly evaluated.

Our ADHD evaluation is specifically designed to work through this differential carefully. The goal is not to confirm a self-suspected diagnosis. The goal is to establish what is actually true — and build a treatment plan from there.

The Evaluation Process

What a Structured ADHD Evaluation Actually Looks Like.

Our ADHD evaluation is not a checklist. It is a structured diagnostic interview that systematically examines your symptom history, functional impact, and the alternative explanations for what you are experiencing before arriving at any conclusion.

About Psychiatric Evaluation
Provider conducting an ADHD evaluation with a patient — D&Y's thorough assessment process
  • Symptom history and onset

When did attention, focus, and executive function difficulties begin? Were they present in childhood, even if unrecognized at the time?

  • Functional impact at work and home

How are your symptoms affecting your job performance, relationships, household management, and overall quality of life?

  • Differential diagnosis

Active ruling out of anxiety, depression, bipolar disorder, sleep disorders, burnout, and other presentations that produce symptoms closely overlapping with ADHD.

  • Personal and family psychiatric history

ADHD has a strong genetic component. Family history is a meaningful clinical data point that informs diagnosis and treatment direction.

  • Prior treatment history

What has been tried before? Did it help? How was the response to medications, if any were previously prescribed? Prior treatment history is clinical data, not a starting point to work around.

  • Current life context

High workload, major life transitions, and chronic stress can produce ADHD-like symptoms in adults who do not have the disorder. This context is evaluated carefully, not dismissed.

  • Cognitive and behavioral patterns

Detailed discussion of specific patterns: time blindness, hyperfocus, emotional dysregulation, working memory difficulties, and task initiation problems that distinguish ADHD from other presentations.

  • DSM-5 diagnostic criteria

Formal application of diagnostic criteria with clinical judgment — not just a self-report questionnaire handed to you in a waiting room.

Our Prescribing Approach

Conservative, Monitored, and Always Explained.

If evaluation confirms ADHD and medication management is clinically indicated, D&Y follows a structured, conservative prescribing approach built on three principles.

Diagnosis Before Prescription

Medication is not prescribed before a thorough evaluation — not based on self-reported symptoms alone, not based on a prior chart diagnosis without review. A proper, current clinical diagnosis comes first, every time.

Minimum Effective Dose

D&Y uses the minimum medication necessary to achieve meaningful improvement. He does not escalate doses quickly, does not stack medications without clear indication, and evaluates medication response at every follow-up using structured criteria.

Full Transparency

Every medication decision is explained — what it is, why it is being recommended, what to expect, what timeline to give it, and what the alternatives are. You are involved in the decision. You are not handed a prescription at checkout.

After the Diagnosis

ADHD Management Is Not a One-Time Prescription. It Is an Ongoing Process.

Getting an ADHD diagnosis is not the end of the process — it is the beginning of it. Effective ADHD management requires structured follow-up to evaluate medication response, adjust dosing as needed, track functional improvement over time, and address the anxiety, sleep problems, and mood symptoms that frequently co-occur with ADHD.

D&Y schedules follow-up visits at 2–4 week intervals early in treatment, when medication response and side effects are most variable. Once stable, visits transition to monthly, then every 1–3 months for maintenance. Every visit includes a structured review of how things are actually working — not just a refill.

For patients who benefit from both medication and therapeutic support, D&Y coordinates actively with therapists and can integrate supportive techniques within the psychiatric appointment when appropriate. Learn more about our combination therapy approach.

Follow-Up Schedule

Week 2–4

First Follow-Up

Medication response check. Any side effects? Any early signs of improvement? Dose adjustment if clinically warranted — not by default.

Month 2

Stabilization Check

Is the medication producing meaningful improvement in focus, executive function, and daily life? Are treatment goals being reached? Adjustments made with intention.

Month 3–4

Maintenance Transition

Shift to monthly visits as treatment stabilizes. Continued monitoring and refinement. Co-occurring symptoms — sleep, mood, anxiety — reviewed at each visit.

Month 4 and Beyond

Long-Term Management

Quarterly visits for stable patients. Ongoing review of functional goals. Adjustments made as life circumstances, workload, or life transitions change.
